Health Insurance Health Insurance is one of
your largest business expenses. ASG offers a complete line of Health Insurance
products from HMO's to self-insured plans. We have exclusive provider
arrangements with regional carriers as well as the new Consumer Driven Health

Dental Insurance ASG has an exclusive trust
arrangement for our clients through Delta Dental.This provides discounted group
dental plans using the Delta USA network. There are three plans available: two
traditional plans that have participation requirements for the group, or a group
voluntary plan that can cover a minimum of only three employees with no minimum
participation requirement. Plan availability varies by state. Please contact an

Life Insurance ASG has a
program underwritten by CNA that offers our client's very competitive rates and
benefits. We can offer your employees a flat amount of group life insurance or a
multiple of salary. In addition, dependent life insurance is also available for
spouses and children. return to top
Short-Term Disability Insurance This provides
your employees with income replacement in the event of a non-work related
illness or accident. Benefits can start on the 1st, 8th, 14th or 30th day of a
disability and continue for six months. The benefit amount is usually 60% of
salary. This is a very important coverage given the fact that most families do
not have enough money put away if their paycheck stops because of an accident or

Long-Term
Disability Insurance This coverage usually begins after 30, 60,
90 or 180 days (employer's choice) of a disability and continues until the
disability ceases or age 65 occurs. This coverage is usually combined with a
short term disability plan, either insured or self-insured. return to top
Vision
Insurance Our Vision coverage can be purchased on a stand-alone
basis or tied to other coverage depending on group size. We offer the
traditional Vision Service providers as well as self-insured Vision plans. return to top
Supplemental
Products Supplemental products are fast becoming a major
employee benefit. Supplemental products are usually 100% employee paid and
include Life Insurance, Accident coverage and Cancer/Dread disease coverage.
These plans are permanent and portable for your employees and can enhance your

Section 125
Plans Section 125 plans come in two forms. The basic 125 plan
allows your employees to pay their portion of their benefit costs on a pre-tax
basis. The full 125 plan adds Medical reimbursement and Child care accounts to
the basic plan. These accounts allow employees to pay certain medical and child
care expenses also on a pre-tax basis. return to

Key Person Life Does
your company have key people? This is a person whose death would adversely
affect the earnings of your business. This policy is owned by the business and
the business is the beneficiary. We have arrangements with several companies

Retirement Plans Supplemental Retirement plans are non-qualified
plans that can be purchased to supplement current qualified retirement plans.
The typical 401(K) plan doesn't do the complete job when saving for retirement
